PLEASE NOTE: This form is to register teens for the off-site activity. No registration or payment is required for teens to attend the Youth Sessions on Tuesday and Wednesday mornings at Selah Conference.

OFF-SITE ACTIVITY DETAILS

WHERE: Aqua Warrior Inflatable Water Park, Clontarf (103 Hornibrook Esplanade, Clontarf QLD 4019)

WHEN: Tuesday 14 April, immediately after the morning conference sessions

COST: $30pp (this covers lunch AND water park entry)

BRING: Swimming clothes (shirt and shorts required), towel, sunscreen

DROP OFF: Good Shepherd Baptist Church @ 12:30pm

or, meet the group at Aqua Warrior at 1:30pm

PICK UP: Good Shepherd Baptist Church @ 4:30pm

or, be picked up from Aqua Warrior at 3:30pm

TRAVEL

The Selah Youth leaders will be transporting teens to and from Aqua Warrior via their personal vehicles and the church/school bus. If you are not comfortable with this arrangement, please organise alternative transport for your teen/s - they will need to be dropped directly at Aqua Warrior to meet the group at 1:30pm and picked up from Aqua Warrior at 3:30pm.

MEDIA RELEASE

While Good Shepherd Baptist Church aims to exercise reasonable care during the running of this event, I hereby release Good Shepherd Baptist Church and its leaders from any liability whatsoever arising out of injury, damage, or loss that may occur.
